Youth Home

Horizon House is a traditional Community Residential Setting (CRS) located in Richfield, Minnesota, designed to provide a structured, supportive environment for children who benefit from group home–level care with access to individualized support.

This four-bedroom home offers each child a private bedroom and the opportunity to remain actively engaged in their local community, including attending school and maintaining important connections. Horizon Home is not a crisis stabilization program, and length of stay is individualized based on each child’s unique needs and progress.

Program Details



  • Serves youth ages 5-17


  • Serves all genders


  • Accepts CADI Waiver and private pay


  • Referrals must be completed by a case manager, social worker or hospital social worker
